Rehabilitation

The council’s Social Services department and our NHS partners are committed to enabling vulnerable, frail or disabled people to maintain their independence in the community for as long as they want to, rather than having to go into residential or nursing home care in order to receive the care they need.

Sometimes after illness or injury people find themselves needing extra help in order to regain their confidence or ability to manage at home. This is what we describe as ‘rehabilitation’ or sometimes ‘re-enablement’.

Rehabilitation aims to:

  • rebuild your skills and confidence in doing everyday activities
  • teach you new skills to help you manage at home
  • keep you out of hospital, or permanent residential care, unless this really is the right choice for you.

There is a wide range of services available, including:

  • rehabilitation services for people who have suffered a stroke
  • a ‘falls clinic’ for people who have suffered a fall and need help to minimise the risk and fear of further falling
  • intensive home care support
  • support through the District Nursing Service
  • community based and short term residential intermediate care services – providing high levels of focused support for a short period to help avoid admission to hospital or to residential care, or to help ensure that people don’t stay in hospital unnecessarily
  • Occupational Therapy services – help with regaining mobility, or with equipment, aids and adaptations which will help people regain and retain their independence
  • specialist day services for older people suffering from dementia
