Single Assessment Process for Older People

The single assessment process (SAP) means older people will not have to complete lots of forms when they need different legal and social services. Its purpose is to ensure that older people receive appropriate, effective and timely responses to their health and social care needs, and that professional resources are used effectively.

If you have any questions about SAP in the borough, please contact the SAP Project Administrator on  020 7364 2195.
