Services for vulnerable people
This service is for adults who are considered ‘vulnerable’
within the community of Tower Hamlets where there are risks to
their health & safety, autonomy, management of daily routines
& involvement in family or community life.
Where people are eligible for support we are able to provide
care packages & ongoing case management as well as referral to
other agencies such as occupational therapy & tenancy support
services.
Who is the service for?
Adults aged between 18-65 years.
Residents of Tower Hamlets.
Those who do not meet the specialist criteria of other adults
health & well being teams but who do have social care needs
& who have critical or substantial needs following a community
care assessment under the national assessment framework guidelines
‘Fair Access To Care’ .
How to access the service.
Following a referral to our team, a social
worker will contact you & offer you a community care assessment
which will normally take place in your home.
Your presenting needs will be discussed & following the
outcome of this & where eligible, services will be offered. We
can arrange care packages whereby care workers can come into your
home to provide you with assistance for your personal care &
domestic care needs. Following the assessment we may also refer to
other services.
Who to contact
Referrals for ‘vulnerable adults’ should be
made via the social care team (0207 364 5005).
Referrals for clients wanting to access
de-tox/residential treatment centres for their drug misuse are
accepted via fax directly from third party community drug services
(for example GP, community drugs team, specialist addictions unit).
Duty can be contacted from Monday to Friday 9am –
5pm.
