Single assessment process
The purpose of the Single Assessment Process (SAP) is to ensure
that older people receive appropriate, effective and timely
responses to their health and social care needs, and that
professional resources are used effectively. The objective is to
use one common assessment tool and share assessment information, to
avoid duplication of time and effort.
SAP also encompasses joint-working in multi-disciplinary teams
and working across organisational boundaries - concepts that are
very much aligned to National policy guidelines set out in the
recent White Paper on improving community health and social care
services.
Four organisations are involved and committed to the project:
Local Authority, PCT, Mental Health Trust and BLT.
Any access points for those organisations offering Older
People’s Health and Social care services will offer a SAP
assessment.
