gym. A specialist exercise referral service for people with
disabilities or long-term health conditions. You get one to one
exercise sessions with an advanced instructor who supports you to
set and reach your fitness goals.
Who is it for?
Primarily people with disabilities or
long-term health conditions for the exercise referral service, but
the gym is open to all.
To use our services
Referrals are welcomed from GPs,
physiotherapists, occupational therapists or other clinical health
professional. Referral forms can be downloaded from our website

You can self-refer but we always need to have
a referral form completed by your GP. Any queries about the

Bondhon Women's Project
The Bondhon Project aims to empower
individuals by building their self-confidence and help them to
integrate back into the community through individual support and
activities such as exercise sessions, cookery and health
information talks.
Bondhon runs two weekly group sessions
–
(1) A transport-assisted session at St
Hilda’s community centre on Tuesdays, including a regular exercise
class, lunch and quarterly arts and craft workshops.
Use of St Hilda’s free Advice Service as
well as other service provided at the centre.
(2) A session at Around Poplar Sure Start
Children’s Centre on Mondays, targeted especially at mothers with
young children who are experiencing mental health problems such as
post-natal depression.
A four week be-friending programme for women and their
family who need the added support to start attending
activities.
Who is it for?
The Bondhon Project promotes the well-being
of Bangladeshi women who are socially isolated and experiencing
mental health problems, through group activities and one-to-one
support/befriending. The project caters for women living in the
Tower Hamlets area aged between 18 to 65 years.

This service is free to eligible users. For personal

Children’s Direct Payments Service
You can have more say and more control over
your child’s life by using something called a direct payment to get
money from the council and decide where and when you use it to keep
your child safe, healthy and well.
Our Children’s Direct Payments Service can
help you decide what kind of care your disabled child needs most,
and help you apply for money from the council to pay for it.
Getting a direct payment won’t affect your
benefits.
Don’t worry if you’re a bit nervous about
managing the money. We can help with everything, including finding
out what care is available, working out the costs, setting up a
bank account and helping you keep track of the money that’s
spent.
We have staff who speak English, Somali,
Bangla and Syleti and can give you information in person, over the
phone, by email and in paper, audio and electronic formats. So you
can get help whatever way you need it.

disabled children who live in Tower Hamlets.

The Community Learning Disability Service (CLDS) provides
specialist health and social work assessments, interventions, care
packages, therapies, advice, training and consultation for people
with learning disabilities who are 18 years and over (age limit
under review). Also as part of our service delivery we work

Community Options - support, advice and recovery service
(mental health)
This service is commissioned by the Primary
Care Trust (PCT) to enable and empower those in the preliminary
stages of mental health needs. Our aim is to provide practical and
emotional support and prevent those who are suffering from mental
health problems escalating to severity, to work towards recovery
and lead valued lives in the community. We provide English,
Bengali-Sylheti, Somali, Hindi, Urdu and Arabic speaking support,
time & recovery workers who can support you on a one to one
basis through your current circumstances.
We will provide a thorough need assessment and
risk assessment, a support plan of what and how to achieve the
goals in order to maximise your independency and signpost you to
the appropriate agency as per need. We will measure your progress
by using a psychometric tool called Recovery Star, a visual star,
easily understood by both the client and the ST&R Worker to
focus on the needs and endeavour to achieve an outcome agreed on
the onset of the assessment. We also promote Person Centred
Planning (PCP) and Wellness Recovery Action Plan
(WRAP). This is a short term service, minimum of 3
months and maximum of 12 months, depending on the need and the
intensity of the illness.
Who is it for?
This service is for people who are suffering
from stress, anxiety, depression or any other form of mental
illness. We provide 1 to 1 practical and emotional
support. The service is for Tower Hamlets residents aged 18
years or above.
To use our services
We will receive referrals from GPs, Primary
Care Mental Health Workers, Primary Care Psychology services,
Community Mental Health Teams and self referrals.

Community Options - service user involvement
project
We co-ordinate a quarterly open service
user forum called ‘Your Say, Your Day’ which is lead by the needs
and requests of the local community affected by mental health. We
support and develop 24 varied service user-led groups, as well as
encourage new groups and recruits. Empower and support people to
use their valued experiences to influence and to become involved in
the planning, development and implementation of local services. We
work with the statutory and voluntary sectors and local agencies to
promote social inclusion, involvement and recovery. Provide
information on rights, events, training, activities, jobs,
promoting and enabling peer support and community involvement.
Support and encourage people to learn new skills and to utilise
their skills to support and train others. We also provide a range
of training opportunities.
Who is it for?
The service is for people who have mental
health experience living in the community who want to get involved,
have something to say, a story to share, are feeling bored,
isolated or forgotten about. Have ideas and opinions and want to be
involved in the planning and delivery of local mental health
services. Wish to develop skills and increase their confidence to
make informed choices and changes for the better. Wish to start or
join a service user-led group.

Family Action Building Bridges
Family Action Building Bridges Services works
with parents/carers with enduring mental health difficulties
including bi-polar affective disorder, schizophrenia, post
traumatic stress disorder and depression-related illnesses who have
responsibility for the well-being of children. We provide a
preventative service which seeks to maintain the care of children
in their own families.
We offer:
- emotional support
- promoting positive attachments between family
members
- practical support i.e. help with benefits,
housing issues
- financial support
- advocacy i.e. support with CPA meetings, CORE
meetings
- activities for children
- signposting to other services
- support with parental skills ie boundaries,
building self esteem and positive relationships

Service users and their children are seen once
a week for two hours. Workers will dedicate one hour to the parent
affected by mental health needs and one hour to the children. The
support will be provided for up to a year.

Parents / carers with enduring mental health
needs, their carers and children.
To use our services
Referral must be made by a Community Mental
Health Worker from the four main community mental health teams in
Tower Hamlets.
If in doubt about whether a referral is
appropriate you can contact the Building Bridges Co-coordinator who
will be happy to answer any questions you may have. Referrals will
not be accepted if children are on the Child Protection Register or
have a Child Protection Plan.

Family Action family visiting service
Family Action liaises with the Mile End
Hospital to provide a safe, child-friendly environment for children
and young people to meet their parents and relatives during the
parent’s or relative’s stay in hospital due to mental
illness.

The family visiting room is available to any

Grounded Ecotherapy, Providence Row Housing Association
Grounded Ecotherapy is an Ecomind’s funded
therapeutic horticulture project. Based in Tower Hamlet’s Cemetery
Park, we provide the opportunity to learn about and be actively
involved in conservation activities. This includes tree felling,
brush clearance and native wild planting. Aside from the practical
aspect of the group. There is a chance to socialise, improve
physical health and benefit from working outside in a beautiful
woodland. We provide a cooked vegetarian meal at lunch time. There
is also opportunities be receive AQA accreditation
certificates.
Who is it for?
Grounded Ecotherapy is a mental health and
well-being project, aimed at adults who have experienced mental
health problems. Additionally, we accept people who are in recovery
from co-occurring substance misuse problems.

Mind
in Tower Hamlets and Newham
Counselling service: The counselling services
at Mind in Tower Hamlets and Newham provide weekly individual
counselling. The sessions last 50 minutes and are an ideal
opportunity for you to focus on your issues in a safe and
confidential environment. The service adheres to BACP guidelines
for ethics and good practice in counselling and psychotherapy. We
operate a charge for our service which is as follows: £5 for
people on benefits, £10 for people working part-time, and £15 for
full time workers. If you would like to make a referral please
contact the counselling manager or counselling assessor on 020 7510
4248/7.
Welfare benefit advice:
Casework is provided on all welfare benefits, including
applications, advice on rights and obligations regarding benefits,
request reviews, reconsiderations, appeals for tribunals,
overpayment issues, local government issues (housing or council tax
benefit) , better-off calculations in case of income from
employment, social fund applications and reviews, communications
with DWP, Job Centre Plus and local councils, medical assessment
forms, working and child tax credit issues, representation for
debts under £5,000 and planned consolidation as well as advice on
debts over £5,000.
Mental health recovery and support
services: Supporting people with mental health issues to
towards recovery and remain well in the community. This
generic service provides intensive one to one support using the
recovery tool devised by Mental Health Providers Forum.
- Using the Recovery Star Support service users
in crisis and who need to be supported to access services in a more
planned way.
- To support service users who have severe and
enduring mental health issues and who lack awareness of the
clinical pathways into services.
- Casework and work in partnership with
assertive outreach Service, CMHT’s, THIES and the Dual diagnosis
Team to engage with service users to support and promote recovery
in the hospital and the community.
- Encourage service to access
therapeutic/mental health wellbeing activities available in Open
House Mind in Tower hamlets and Newham.
- To support people with complex needs who are
linked to the Assertive Outreach service, CMHTs, Dual diagnosis
services.
- Facilitate users’ access to relevant
specialist services, both in the community and in the hospital
- Offer support to people who are newly
admitted to services and who are not sure of the range of services
open to them.
- To support clients to access advocacy and
counselling services.
- Provide information to users, carers and the
community regarding mental health services and resources, treatment
options and aftercare; complimentary to that which statutory
services are bound to provide.
- Provide information to users on healthy
living style, e.g., healthy diet, exercises, stop smoking
Complementary support:
Traditional Chinese medical massage or acupuncture, homeopathy,
music therapy, art therapy. There is charge of £5 or £7 for Chinese
therapies. There may be a cost involved for homeopathic
remedies.
Social engagement: There are
Out of Hours services on Monday and Thursday evenings with
activities and support workers. A meal is normally prepared
together with users of the service. There is a separate regular
Sunday activity group, which often goes out to different activities
in and around the borough. A range of activities take place Monday
to Friday during the day at Open House, such as storytelling and
poetry group, photography group, cycling, badminton, and movement
therapy.
Independent advocacy: We
provide community and inpatient advocacy service for both clients
who are informal (Inclusive) and those who are detained under the
mental health act (IMHA). This service is available from
within the mental health service at Mile End Hospital and from our
main office at 13 Whitethorn Street.

Our services are for people who live in Tower
Hamlets and Newham, who are 18+, and for people who have experience
of mental health and emotional issues.

Pritchards Road day centre
Our group activities and programmes promote
good health and mental well-being, and are aimed at supporting
individuals through their recovery, promoting social inclusion, and
reducing isolation. We aim to support individuals in their personal
development and recovery through facilitating a range of skills
based programmes alongside physical health activities and
therapeutic groups. The service works to support individuals in
accessing education, training and vocational opportunities, leisure
pursuits as well as other community resources. The centre
provides a range of structured day activities and a drop-In on two
afternoons a week.
Our projects and groups are as follows:
Vocational projects: gardening and
horticulture, all sorts shop retail project, tea bar catering
project and IT.
Lifestyle and therapeutic groups: weight
awareness, art, music, drama, Bengali men’s group, mosaic cultural
group for African, African Caribbean and Asian people, massage,
dance and movement, soccer fitness and other physical health
activities (these may vary), pottery, acupuncture.

Who is it for?
Pritchards Road groups and projects are open
to adults experiencing mental health problems between the ages of
18 – 65 and living in Tower Hamlets.
Any of the following people can refer you,
care co-coordinator, nurse, GP, support worker or psychiatrist.
However you must have a care co-coordinator or be care managed.
To use our services
Your care co-coordinator/support worker can
arrange for you to attend an informal visit, this will allow you to
visit the centre and find out more about the service we provide.
From this both you and your care co-coordinator will need to
complete a referral form (which you can get from the centre). We
will also need a copy of your care plan and a risk assessment.
Once all of these documents have been received
the team will be able to make a decision based on the information
given. If you meet our referral criteria you will be invited in for
an initial assessment and will be informed of the outcome within 2
weeks of your assessment.

Sundial Centre Outreach Service
Sundial Centre’s friendly outreach worker
offers support to people over fifty living in Tower Hamlets to
maintain and improve their quality of life. Alys works with clients
to enable and empower them to maintain good physical and mental
wellbeing, in clients’ homes, at the Sundial centre, or at a
location convenient for the client.
Our flexible outreach service can help with a
wide variety of issues, such as filling in forms, accessing social
services, benefits advice, housing issues or support to find and
access other services and activities.
Alys also runs a number of groups open to all
over 50s. Recent and current groups include a weekly men’s
group at the Sundial Centre, a fortnightly depression support group
for at St Josephs hospice and a ‘preparing to go home’ group for
people who are inpatients at Thames House, Mile End Hospital.
You can pop along to see Alys at one of her
regular surgeries in a variety of locations, such as sheltered
housing schemes, Ability Bow, One Stop shop/s, the Job centre and
every Monday at Sundial.

Our popular Sundial Café is open to everyone 5
days a week 12 midday to 1.30pm serving delicious, good value,
freshly cooked food and our coffee bar is open for refreshments
from 10am to 3pm.
All of our members are encouraged to use our
full range of services, to get involved in decision making and we
welcome volunteers, young and old. As well as all our drop in
activities, we run a range of other projects, including: Day
care service for over-65s. LinkAge Plus: group activities and
outreach to older people their homes (delivered by five
organisations across Tower Hamlets). Outreach support for isolated
older people. Learning zone, which provides employment advice,
training and qualifications

Who is it for?
Most of our activities are aimed at people
over 50. Our Learning Zone is aimed at anyone over 18. We always
welcome younger adults to the centre and would love anyone to get
involved.
To use our services
We welcome all adults at Sundial, particularly
over 50s. Most of our services do not have eligibility criteria and
all we ask is for a short registration form when you first
arrive.
What is the cost?
The centre is free to drop in at anytime
during opening hours. Some groups/activities may charge a small fee
to cover costs (avg price of £1).

Vietnamese Mental Health Service
To provide a culturally sensitive service,
through group support and activities, and one to one support
Vietnamese and Chinese men and women in Tower Hamlets, experiencing
mental ill health and social isolation in order to promote their
Wellbeing.
1 to 1 support: A
keywork process by using Recovery Star approach to support clients
to build a satisfying and meaningful life: One-to-one sessions (of
2 hours on average including face to face work, follow up and
contact), providing support to service users using recovery star
tool and working closely with CMHT. Providing advocacy and language
support to service users at the initial assessment and diagnosis
meeting with psychiatrist; or review meetings assessing their
mental stage and medication. Supporting service users at their CPA
Care Plan review meetings. Joint visits with care co-coordinator
providing advocacy and language support in the monitoring service
users mental stage; ensuring service users are compliance with
medication and progress of treatment. Hospital visits,
attending ward-rounds, providing advocacy and language support to
psychiatrist with interpreting, supporting service users to raise
any specific concerns. Providing advocacy and language support to
service users at their GP appointments to ensure their physical and
mental health needs are being assessed. Home visits to assess
service users and their cares/family health and social care needs,
providing practical help, e.g. filling in welfare benefits and
housing benefit forms, reading any letter received, and providing
mental health awareness information to promote mental well-being.
Facilitating users’ access to relevant specialist services in the
community to prevent social exclusion.
Drop-in/day care (3 hours each session):
Providing service users and carers a place for socialisation,
mutual support and opportunity to learn daily living skills.
Providing staff support to individual enquiries, e.g., reading
official letters. Providing culturally appropriate leisure
activities e.g., Vietnamese & Chinese magazines, books and
games. Refreshment, hot lunch. ESOL or painting
classes. Health education, talk/presentation.
Mental health workshop &
health newsletter: To hold quarterly mental health
workshops / seminar open to all Tower Hamlets Vietnamese and
Chinese communities to raise mental health awareness in the
communities and professionals working with this cultural group.
Topic may include discussion on physical health, mental health
issues, healthy eating and healthy living style. To publish and
circulate to the Vietnamese and Chinese communities in Tower
Hamlets a minimum of 6 issues a year, written in English, Chinese
and Vietnamese, contents included: health news worldwide; health
news in Vietnam; specific illness cause and treatment; questions
& answers of specific illness and medical stories.
Who is it for?
Vietnamese and Chinese mental health users and
their families.

Referrals are accepted from GPs,
psychiatrists, community mental health teams; voluntary and
community organisations and self-referral. Referral can be

What is the cost?

budget:
1. Package of care for service user and
his/her whole family - £2,700.00. This provides 12 months
specialist care package for the Vietnamese / Chinese people with
severe mental health problems. The package will provide 50
hours of one to one support and 48 sessions of day centre
activities as above; plus 4 health workshops and a minimum of 4
issues of health newsletter
2.1 to 1 support session (on average 2 hours
per session): £30 per hour
3. Drop-in/day care session: £25 per person/per session

Working Wonders Bromley by Bow Centre
Working Wonder provides:
1. PoLLeN (People, Life, Landscape and Nature): A range of
horticultural and environmental activities for people with mental
distress. Activities include allotment growing, flower growing and
arranging, park maintenance, art and design in the landscape,
healthy eating and motivational coaching.
We welcome individuals who are affected by
stress, anxiety, panic attacks, agitation, low mood and with low
self confidence.
2. Artychoke: A range of art and design
activities for people with learning and physical disabilities,
working with the elements of landscape and nature to develop
products for sale. Participants are provided with the opportunity
to work collaboratively with artists to design and produce a range
of ceramic, printed linen, photographic and other hand-designed
products with an aim to enhance confidence and skills.
3. PEACH (Personal, Enabling, Arts, Choice,
Health): Supportive and creative arts activities led by
professional artists for people with learning and physical
disabilities, including pottery, mosaics, textiles, sculpture,
multimedia, dance and movement.
4. Arteast: A visual arts enterprise for
people with learning and physical disabilities. Participants can
work alongside artists and volunteers to make unique merchandising
products and public art works. Participants are provided the
opportunity to work for businesses and organisations in printing
cotton bags, T-shirts, banners and badges to enable them to promote
themselves. There may also be opportunities for them to work to
commission in creating 3D models and installations for festivals,
parties, shop windows and internal decorative displays. Arteast
seeks to develop creative, social and business skills for
participants. An employment programme is in place for participants
and the whole group is encouraged to contribute to all aspects of
creative and business decision making.
5. SAGE: Offers a range of creative, health
and social activities-based programmes for the 60+ in the
community. It is designed to increase and maintain participants’
independence and provide activities in a cross-generational and
supportive environment of the Bromley by Bow Centre.
Activities include:
- Young@Art: Art-based workshops, reminiscence
work, outings and events. Activities are led by professional
artists and include painting, model-making, silk-screen and
drawing.
- Healthy Lifestyles: Weekly physical
activities for people with chronic pain, long term illness or
disabilities to increase movement and activity levels.
- Bow Diners: Free healthy meals in partnership
with Foodworks. It is open on Sundays for everyone over 60 years
who would like to come and meet others over lunch.
6. My Weigh: A free one-year weight management
programme for overweight adults living in, or registered with a GP
in Tower Hamlets. The programme assists participants to achieve
long-term weight loss and sustain healthier lifestyles.
7. Your Home Your Environment: Home care
service supporting individuals to maintain independence within the
comfort and security of their own home.
Who is it for?
Our diverse and integrated range of services
are for people with learning and physical disabilities, mental
health needs, chronic health conditions and older people.
To use our services
Individuals can: Come to us with their own
personal budgets. Be referred by friends and family. Self-refer. Be
referred by their GP and Health Professionals. Be referred by any
statutory/ independent/ voluntary service providers.
