Amonet Limited
Our services include personal care (washing,
toileting, dressing etc), housework, shopping, collecting pensions
and prescriptions and paying bills and befriending. We can
support you to make or order healthy meals, attend social and
leisure activities, visit the hairdressers or attend a course.
Our Hospital Discharge Service and support to
hospital and doctor’s appointments will help you stay
healthy. If you don’t need regular services but just want
peace of mind that someone is there to check in on you from time to
time during the night and respond to an emergency call, consider

Chinese Association of Tower Hamlets
Our services are designed to give the help
and support which you need to carry on living in your own home. We
identify these needs when we visit you to make an assessment. The
list below gives you an idea of the type of tasks which we can help
with.
Personal Care: getting out of bed, getting dressed/undressed,
bathing/showering, going to the toilet/managing incontinence,
prompting to take medication, sit-in service.
Domestic Care: shopping, laundry/ironing, making bed/changing

Before we take on any new package or any new
clients we usually agree on a suitable day with the client and
their representative/family member to discuss with the clients what
their care needs are and agree on a suitable care package.
Upon agreeing on a suitable care package we
with then carry out a risk assessment of the property, the client
and every equipment that will be used in delivering the care.
During the assessment, we usually have the client as well as their
family member/representative present. If any risks are identified
during the assessments we discuss ways of minimising the risks to
avoid accidents to both the care giver and the client. Risks
assessment is very important and we DO NOT provide care until this
has been carried out and this is done within 48 hours upon
referral.

Shebadan Domiciliary Care - St Hilda's East
Community Centre
We provide culturally appropriate
domiciliary care for service users suffering disabilities and are
unable to carryout their personal hygiene, domestic tasks etc. The
service users we provide services for are homebound and from
a predominantly Bangladeshi/Muslim background though the service is
open to users of all races/creeds. Amongst services we provide

- Provide users handbook that contains all the necessary
records keeping materials, Policy and procedures (both in English
and Bengali)
- The Team Leader ensures that the user is fully informed
about the service specification.

Tower Hamlets Reablement Service
The Reablement Service provides independence building support
(for up to six weeks) to optimise a person’s functioning in all
aspects of daily living, supporting an improvement in their
physical, social, mental and emotional wellbeing.
Independence planning and service delivery may
include the prescription or provision of assistive equipment and/or
minor environmental adaptations, advice and support on access to
activities in the community, and Reablement Officer input in your
home to support you to achieve goals during the six week
period. The team includes staff from a range of professions;
for example Occupational Therapists, Social Workers, Nurse
Advisors, and Independence Planners.
The Reablement Service will also provide adult
safeguarding functions when required, support and assessments for
needs of carers, and where appropriate undertake assessments to
determine ongoing community-based support needs, and calculation of
an indicative personal budget prior to an individual starting the
Support Planning process.
It is intended that people using
the Reablement Service will experience:
- Services that will robustly promote independence and
access to a range of preventative and personalised interventions
that will help people improve their confidence and ability to
participate in activities and roles that they value
- Good quality information about what is available from
the Council and what is universally available
- Acquisition of skills to access services independently
or with an appropriate level of support required
- A minimum of handoffs
Who is it for?
This service will tend to be provided to people who have
just been discharged from hospital or are otherwise entering the
care system following a crisis. Eligible adults are those
who are ordinarily resident in Tower Hamlets. In the case of
carers, support will be provided if the cared-for person or people
are ordinarily resident in Tower Hamlets.
Exclusions include :-
- End of Life care – in cases where a user is in ‘terminal phase’
of illness
- Those users more appropriate for Community Based specialist
services e.g. Rehabilitation Services for physical and/or
mental health needs
- People under the age of 18

Referrals to the service can be made via the following routes

If you are in one of the boroughs local
Hospitals (Royal London, London Chest, Mile End Hospital) you can
be referred directly to the service via the First Response Hospital
Social Work Service based at these hospitals.
If you are living at home you can refer
yourself via the Social Care First Response Service on 020 7364
5005 or adultcare@towerhamlets.gov.uk
In both cases these teams will initially
assess your needs and decide if a referral to the Reablement
Service is appropriate.
