Transition
Transition refers to the process by which
young people move from being supported by children’s services to
being supported by adult services.
Tower Hamlets is committed to offering all
young people with continuing support needs a person centred,
transparent and timely transition. It is time when young people are
nearing the end of their schooling and next step plans are needed
to identify continuing education, training, employment, housing,
health and social care support and leisure.
Transition planning begins when the young
person turns 14 and a plan is formulated with the help of several
key people such as teachers, family and social workers to help
young people decide what they will do when they leave school.
Adult and children's services work together
during this time along with the young person and their family to
help identify options available that focus on the young person’s
aspirations and considers their wishes at all times.
