Safeguarding Adults Review

The key purpose for undertaking a Safeguarding Adults Review (SAR), previously known as a Serious Case Review, is to enable lessons to be learned from serious injury or death of an "adult at risk" and for those lessons to be learned widely and thoroughly and to reach an understanding of what needs to change to reduce the risk of further such tragedies.
