Community and day centres

Tower Hamlets council has several day centres within the borough, Poplar Day centre for older people, Russia Lane for people with dementia and Mayfield House for people from the Somali community. Sonali Gardens is for the older Bangladeshi community, Sundial day centre is for older people, and St Hilda’s weekend Bangladeshi day centre. They assist in supporting people to live their lives at home and within the community. 

Referrals to this centre can be made following an assessment where a need was identified at a certain level for social, cultural and community support as well as practical assistance with personal care and also to provide respite for informal carers. Also some centres can provide assistance with translation and with legal, housing and benefits advice.

All directly provided day services operate from 9am to 5pm on weekdays with two sessions each day. In order to access these centres you will need an assessment which can be arranged through the Social Care team.

Riverside Day Centre

Day Opportunities

Day Opportunities is a 15 place centre. Service users come from a wide range of communities and present with a varied number of long term conditions. The support service users receive is enhanced by the provision of  centre based OT services.

Russia Lane

Russia Lane provides 25 places daily to users who have moderate to severe dementia.

Create Day Centre

Create is an innovative service provision for adults with learning disabilities. It offers a wide range of activities, training programs and provides high-quality support to its users.

Initial Assessment Team

The Initial Assessment service provides a first point-of-contact for all concerns, queries and enquiries relating to adult social care in Tower Hamlets. The team is made up of social work, occupational therapy and other professionals.

The Initial Assessment Team provide advice, information, contact screening, equipment and service provision, urgent response and adult safeguarding functions. The Initial Assessment Hospital Service provides a service to all Tower Hamlets residents who are in-patients at the Royal London, London Chest or Mile End Hospital. This team provide the same services as the Initial Assessment Team, and are also involved in the discharge planning of people from hospital.

Initial Assessment Team can be contacted on 020 7364 5005, 9am-5pm, Monday-Friday. They are based at John Onslow and hospitals in Tower Hamlets.
