The waste service strike is now over and we have restarted service from 27 September 2023.

Over the next few weeks, our waste service team and private waste contractors will catch up on missed collections and street cleaning as quickly as possible.

We thank everyone for their patience and apologise for the inconvenience

Deprivation of Liberty Safeguards: Forms

Tower Hamlets are now using the ADASS/Department of Health DOLS forms.

If anyone, including relatives, friends, carers or any other third party; is concerned that someone is being deprived of their liberty they should contact Tower Hamlets DOLS Service to start the DOLS process.

Managing authorities and supervisory bodies have specific responsibilities of safeguarding individuals in their care and making decisions in their best interests. There is responsibility on these organisations to evidence this by using to the new DOLS process, where they need evidence that they are always taking into consideration their human rights.

To apply for a DOLS authorisation, please see the guidance and forms.

Managing authorities can complete DOLS application forms electronically (urgent DOLS form 1, standard DOLS form 4) and send to dols@towerhamlets.gov.uk by email,  or by fax to 020 7364 7790.

If you are unsure which forms to use, see the Tower Hamlets DOLS Policy and Procedure for further guidance.
