Communities Driving Change

The Communities Driving Change programme is one of the main priorities of the 2017-2020 Health and Wellbeing Strategy.

The positive community-based approach works to build esteem and a shared understanding through active participation. 

Communities Driving Change adopts a community-led approach, in which residents are empowered to take control over their immediate environment. 

The aim is to implement a 'health creation' programme in which residents:

  • identify issues impacting on health and wellbeing that matter to local people
  • recruit other residents who have the energy and passion to make a difference
  • develop and lead new ways to improve health and wellbeing locally

The programme is supported and evaluated by experts from the University of East London (UEL), who have developed a tried and tested model over the past 10 years.

It is a structured programme comprised of the following three phases: 

Phase 1: Scoping (October 2017 to January 2018). Each provider undertook a period of scoping to identify three areas of focus for each locality. Evidence from multiple sources enabled teams to reach this conclusion.

Phase 2: Engagement and action planning (February to April 2018). Engagement processes identifying the priority issues of focus for sustained ‘enabling’ facilitation by the providers. The phase developed an initial action plan of ‘quick wins’ to build momentum within a neighbourhood of action, as well as setting a trajectory for how they will coproduce a response to bigger picture themes identified by citizens.

Phase 3: Delivery (May 2018 to September 2020/21/22)
